Urology Consultants - ESWL

The form on this page can be used to refer patients for treatment at The London Lithotripter Centre. Please download by selecting the ESWL referral button below and print the form, complete, scan and email to lithotripsy@hje.org.uk

Once we have received the completed form a member of our bookings team will contact the patient directly to discuss a convenient time for the patient to attend our clinic for treatment. Following this a member of our pre -assessment team will make contact with the patient pror to their appointment with us to discuss the procedure and perform safety checks with the patient.
